Important notice

We no longer accept paper application forms for registration or practising certificates. Please use our online application process Hapori Matatū instead. Any paper application forms received after 9am on  Friday 29 January, 2021 will not be processed. Applicants will be contacted and asked to resubmit.

Smart forms

  • These forms are designed to be filled in on screen. Printing the form and filling it out by hand might result in your application being incomplete, which can delay the application process as we will need to request further information is provided. 
  • Click on the appropriate link in the table below and download and save to your desktop. You can then open the saved form.
  • Please complete all the sections of the form on screen and save. It is now ready for you to print and sign. Remember to get your referee to also sign it if required. 
  • Send the form with any required supporting documents to the Council using the contact details below.

  • A previously New Zealand registered teacher whose registration expired prior to July 2015
