How to use our forms
Our smart forms are designed to be filled out on a screen so they can be easily printed and sent.
We no longer accept paper application forms for registration or practising certificates. Please use our online application process through instead. This also includes all Limited Authority to Teach applications.

Once downloaded, you can save the form to your desktop.
Please complete all the sections of the form on screen and save
It is now ready for you to print and sign. Remember to get your referee to also sign it if required.
Send the form
Make sure you include any required supporting documents to the Teaching Council .
